Check List:
- Have you signed
all the forms?
- Has your
manager completed and signed the
authorization form?
- Have you had
the documents signed before a commissioner
for oaths?
- If all of the above steps have
been completed;
Fax the forms with you credit card information to 780-462-0633.
If you are paying by cheque, DO NOT FAX, mail all the signed forms with
your cheque.
